摘要:
目的评估经尿道Er:YAG铒激光治疗女性轻中度压力性尿失禁的疗效性和安全性,为临床治疗女性压力性尿失禁提供新的治疗思路。方法选取2017年9月至2018年5月期间86例女性轻中度压力性尿失禁患者为研究对象,分为两组：试验组47例,接受3次经尿道的Er:YAG铒激光SMOOTH模式治疗,每次治疗间隔30 d；对照组39例,接受24次生物反馈电刺激理疗,每周2次,每次30 min,共治疗3个月。分别记录治疗前、后及治疗后3个月的ICIQ-SF问卷评分、1 h尿垫试验、腹腔漏尿点压（ALPP）和最大尿道闭合压（MUCP）,进行统计分析比较。结果经3个月随访,47例试验组患者43例有效（91.5%）,39例对照组患者28例有效（71.8%）,两组间有显著的统计学差异（P<0.01）。两组治疗前后ICI-Q-SF问卷评分、1 h尿垫试验、ALPP和MUCP,均有明显统计学差异（P<0.01）,进一步进行组间分析,试验组的改善程度更优于对照组（P<0.05）。所有患者治疗期间均无不良反应。结论经尿道Er:YAG铒激光治疗女性轻中度压力性尿失禁安全有效,长期疗效仍需进一步观察。
Abstract:
Objective〖WT5”B1〗To explore the efficacy and safety of non-ablative transurethral Erbium YAG(Er:YAG) laser therapy for mild and moderate female stress urinary incontinence(SUI).〖WT5FZ〗Methods〖WT5”B1〗A total of 86 patients with mild or moderate SUI treated during Sept.2017 and May 2018 were enrolled and divided into two groups.The treatment group(n=47) received transurethral Er:YAG laser noninvasive SMOOTH treatment 3 times,with an interval of 30 days.The control group(n=39) received biofeedback electrical stimulation therapy 24 times,twice a week,30 minutes for each time,for a total of 3 months.The results of ICI-Q-SF,1h urine cushion test,ALPP and MUCP were recorded before,after and 3 months after treatment for comparative analysis.〖WT5FZ〗Results〖WT5”B1〗After the 3-month follow-up,the effective rate was 91.5〖WTBZ〗%〖WT5”B1〗 in the treatment group and 71.8〖WTBZ〗%〖WT5”B1〗 in the control group,with a statistically significant difference between the two groups(P<0.01).The results of ICI-Q-SF,1h urine pad test,ALPP and MUCP were significantly different between the two groups before and after treatment(P<0.01).The treatment group showed greater improvement than the control group(P<0.05).No adverse reactions were observed during treatment.〖WT5FZ〗Conclusion〖WT5”B1〗Transurethral Er:YAG laser is safe and effective in the treatment of mild and moderate female SUI,but the long-term efficacy still needs further observation.
